Positive battery terminal melted today! Anybody see this before?

The positive battery terminal has melted completely off, on a brand new battery. Customer said he noticed the old battery had "almost" melted the terminal before he replaced it. Could this just have been a loose connection? I'll check all the connections and the solenoid tomorrow, but would love to hear about any experiences you guys might have had with this before!

What does the end of the battery cable look like?

Perhaps the cable end is defective and creating resistive heat.

Its the original 1999 cable, but it looks fine, no damage from the heat besides slightly melted cover at the end. Little to no corrosion.
